Eligibility Requirements
On the Path Back Home
You should have a say in where you live, and Home Again wants to help you get there. Participation in South Dakota’s Home Again program is open to any adult, age 18 or older that meets the following criteria:
- Is a South Dakota resident.
- Is enrolled in Medicaid or has a pending Medicaid application.
- Resides in a nursing facility, ICF/ID or other qualifying institution for 60 or more consecutive days.
- Meets Medicaid level of care and financial eligibility criteria at least one day prior to transition.
- Will reside in qualified housing upon transition (a home, apartment, or group home with four individuals or less).*
- Is willing to enroll in and can be supported in the community through the provision of an existing 1915(c)HCBS waiver.
- Expresses a desire to live and receive services in a home and community-based setting.

NOTE: Some larger group homes may qualify.

Home Again serves as the bridge for people that have expressed an interest in transitioning back home or to their community. To get started you’ll need to fill out a referral form for yourself or the person you're referring.
