What is Home Again?

Home Again is a program under the South Dakota Department of Social Services with the goal of helping recipients live in the community and setting of their choosing.


When was Home Again established?

Formerly known as Money Follows the Person (MFP), Home Again was established in 2005 to increase access to home and community-based services (HCBS) for Medicaid enrollees.


Is Home Again really designed to help people?

Yes, we move residents that have been in a facility such as a nursing home or hospital back into their own homes or another home of their choosing.


What is a ‘waiver’?

Medicaid programs pay for home and community-based services for elderly or disabled individuals under programs that waive certain restrictions that would otherwise prevent Medicaid from covering services.


How many people has Home Again transitioned nationwide?

Over 107,000 (data from 2008-2020) have transitioned nationwide.


How many days do I need to be in a nursing home before Home Again can help me move home?

60 days, but you can apply to the Home Again program before the 60-day requirement is met.


What if I already live at home?

New guidelines temporarily allow Home Again to help those at risk of institutionalization to remain at home if they qualify and enroll in waiver supports.


Do I have to be confined to a nursing home to move back home?

No, Home Again may be able to assist a transition from an extended hospital stay, such as a swing bed, or other institutions.


How do I make a referral?

Who can make a referral to Home Again?

Anyone can make a referral, including the individual themselves. All referrals are reviewed for eligibility and recipients/guardians are contacted if they qualify.
