Home Again Process

Individuals have the right to know their options, especially when it comes to their health and wellbeing. Home Again’s purpose is to help those who are able transition back home and into the communities they love. If you or someone you know has expressed a similar interest, and meet our eligibility requirements, this program may be the right fit for you.

 

Home-Again-Step-1

Step One - Referral Form

Once the referral is submitted, you’ll be contacted by one of our transition coordinators. They’ll help you review everything the Home Again program has to offer and how it works. Then, you’ll sign a consent form to participate.

Step Two - Entry Screening

Entrance screening helps our transition coordinators identify your goals, needs and the resources necessary for you to get on the path to home.

Step Three - Select A Transition Team

Once you’ve laid the groundwork and are in a good place to make your journey home, our transition coordinators will help you select a transition team to make the move seamless and safe. They will make sure everything is set up properly in your new space, from verifying security deposits to making sure your utilities are working properly.

Your First Year

Your Home Again transition coordinator will remain in frequent contact with you throughout your first year of transitioning. They’ll be there for you to help resolve any problems you may run into or other barriers you need to cross. You will also be asked to complete a quality-of-life survey within your first 11 and 24 months to make sure you’re receiving the care you deserve.
