Our Services

Home Again provides funding for one-time services and supports not typically covered by South Dakota Medicaid. We’ll help you identify barriers, find a place to live and get you the services and supports you need to continue living there.

Transition Services

You have the right to live how and where you want to. With the help of our transition coordinators, we can help you secure a housing unit, set up a new housing unit, or make minor modifications to a current unit.* Once that’s complete, you’ll work with your transition coordinator to identify and select a transition team to help assist you in the journey home.

Relocation Transportation Services

Home Again services will help recipients with relocation services from their current address back to their community.*

Once a recipient is back home, DSS has services to help with day-to-day transportation. To find community transportation you can visit  the Community Transportation Page.

Assistive Technology Services

Home isn’t home if you’re uncomfortable. With the help of Home Again, we can help someone gain access to available supplies and modifications to better support them in becoming independent in self-care. This may cover items such as daily sensors, medication dispensers, grab bars and more.*

Who's Eligible

We want to help remove barriers that may prevent you from returning home or to the community of your choice. If you’re a South Dakota resident that resides in a nursing facility, Intermediate Care Facility or other institution—and you’re currently enrolled in or eligible to enroll in South Dakota Medicaid—Home Again may be able to help.

 

Learn the Process

Home Again serves as the bridge for people that have expressed an interest in transitioning back home or to their community. To get started you’ll need to fill out a referral form for yourself or the person you're referring.
