With Home Again, our partners will help you continue the path to home well after you’ve transitioned back into your community. With waiver programs designed to help individuals with qualifying disabilities, to Medicare planning services that help you get the most out of your healthcare, we’re here to help. Because with Home Again, you’re on your own but not alone.

Partners We Trust

Home Again will assist you in transitioning home, but home and community-based services (HCBS) will help you stay there. These waiver services are available to help you with basic needs, and there are four in South Dakota for which you may be eligible. 

HOPE

Home and community-based Options and Person-centered Excellence, HOPE is for those 65 and older and 18 or older with a qualifying disability.


ADLS

Assisted Daily Living Services (ADLS) are for those with functional limitation in all four extremities.


CHOICES

Community, Hope, Opportunity, Independence, Careers, Empowerment, Success (CHOICES) is a program that provides services to children and adults with intellectual and developmental disabilities. The CHOICES waiver is a comprehensive waiver program with a goal is to assist individuals in leading healthy, independent, and productive lives to the fullest extent possible.


FS 360

The Family Support 360 waiver is a self-directed waiver program that provides participants with an individualized budget used to self-direct their services and supports. Support coordinators assist with identifying natural and paid supports to keep their family together and supported in their home and community.


Dakota at Home

Dakota at Home provides free access to information and planning services to help individuals access public and private services and support in their local community. They can be a key resource in helping individuals stay home after their transition.


SHIINE

Services like SHIINE can help South Dakotans understand the benefits they can receive through Medicare. This free service is confidential and unbiased so you know you’re getting information valuable to you and the future of your health after you transition.
