As a partner of the South Dakota Department of Social Services, we’re able to provide one-time or short-term services that aren’t typically covered by South Dakota Medicaid.

Who's Eligible

We want to help remove some of the barriers that may prevent you from returning home or to the community of your choice. If you’re a South Dakota resident that resides in a nursing facility, Intermediate Care Facility or other institution—and you’re currently enrolled in or eligible to enroll in South Dakota Medicaid—Home Again may be able to help.

Learn the Process

Home Again serves as the bridge for people that have expressed an interest in transitioning back home or to their community. To get started you’ll need to fill out a referral form for yourself or the person you're referring.
